Standing Up a Home Health Program That Kept 200+ Seniors Out of the ER

Willowbrook Senior Living had residents cycling through the ER for issues that, with earlier intervention, didn't need to become emergencies — medication mismanagement, minor wound complications, and mobility decline that went unaddressed between physician visits.

We stood up a full skilled nursing and home-based therapy program embedded directly into the community, with physician-directed plans of care built for each enrolled resident and consistent nurses and therapists assigned to build continuity.

Within the first year, more than 200 residents were enrolled and ER visits attributable to preventable causes dropped by 27%. Willowbrook has since expanded the program to a second community and added our companion care service for residents who need non-medical support between clinical visits.
