VitalCare Health Group wanted to launch a Remote Patient Monitoring and Chronic Care Management program for its highest-risk patients but had no patient-facing technology and no internal bandwidth to build a monitoring dashboard from scratch.
We delivered a cross-platform companion app that pairs with cellular-connected blood pressure and glucose devices, paired with a clinician dashboard that surfaces out-of-range readings in real time. On the operations side, we set up the enrollment, monthly care-plan documentation, and compliant billing workflow end-to-end.
VitalCare enrolled its first 480 patients within the first year, with the program's recurring monthly revenue funding two new care coordinator positions — turning a compliance-driven initiative into a sustainable, growing service line.
