Same-Day Oxygen Concentrator Delivery for a Regional Medicaid Health Plan
A regional Medicaid health plan needed a DME partner that could deliver respiratory equipment fast enough to prevent readmissions. We built a same-day oxygen concentrator delivery program that now covers the majority of member orders.

Heartland Community Health Plan was losing ground on a specific but costly problem: members discharged with a respiratory equipment order were waiting days for delivery, driving preventable readmissions and member complaints.
We built a dedicated same-day fulfillment workflow for oxygen concentrators and related respiratory equipment, prioritizing discharge-linked orders and pairing every delivery with in-home patient and caregiver training so equipment was used correctly from day one.
Today, 92% of respiratory equipment orders are delivered the same day they're placed, with an average order-to-delivery turnaround of four hours. The health plan now cites the program in its own quality reporting and has expanded the partnership to cover mobility equipment as well.
